Qualifications : Health Assistant (HA)/PCL Nursing/CMA qualification under CTEVT or equivalent from recognized institution.
Professional Experience : At least three (3) years of relevant professional experience in providing physiotherapy services to persons with disabilities and in rehabilitation programs

Purpose:
The purpose of a Field Rehabilitation Team Leader is to provide basic healthcare services, support, and health-related guidance to persons with disabilities and other vulnerable individuals in the community. A Field Rehabilitation Team Leader conducts school health programs, facilitates health awareness and counseling sessions, and supports referral cases by connecting individuals with appropriate health institutions and specialized services when needed. In addition, Field Rehabilitation Team Leader works closely with individuals, families, and communities to promote better health practices, improve access to healthcare services, and enhance the overall well-being, independence, and quality of life of persons with disabilities.
Responsibilities:
1. Health Assessment, Basic Rehabilitation Support & Case Management (35%)
- Conduct health and functional assessments of persons with disabilities and identify rehabilitation and support needs.
- Develop basic health support plans and regularly monitor and reassess individuals’ health status.
- Provide basic healthcare guidance, rehabilitation support, and promote safe health and rehabilitation practices.
- Identify and facilitate appropriate referrals and ensure timely follow-up for continuity of care. • Support home modification initiatives and recommend appropriate assistive devices based on individual needs.
2. Community Health Promotion & Disability Inclusion (25%)
- Conduct school health assessments, awareness sessions, counselling, and community education on health, hygiene, nutrition, and disability-inclusive practices.
- Promote positive behavior change and community participation to improve health and well-being.
- Support community meetings, workshops, and awareness campaigns on health and disability issues.
- Advocate for improved access to health services and the inclusion of persons with disabilities in communities.
3. Coordination, Stakeholder Engagement & Family Support (20%)
- Coordinate with the Project Coordinator, Physiotherapist, Social Mobilizers, health facilities, local governments, and other stakeholders to ensure integrated rehabilitation services.
- Provide regular reports on disability rehabilitation and project target, activities and outcomes to the Project coordinator.
- Work closely with persons with disabilities, caregivers, families, and community members to strengthen homebased care and rehabilitation.
- Build effective partnerships and promote collaboration to improve access to quality rehabilitation and health services.
4. Documentation, Reporting, Compliance & Knowledge Management (20%)
- Maintain accurate records of assessments, health services, referrals, and rehabilitation progress.
- Prepare and submit timely reports and project updates to the Project Coordinator in accordance with organizational and donor requirements.
- Document case studies, impact stories, photographs, videos, and lessons learned to support project learning and knowledge management.
- Ensure compliance with organizational policies, safeguarding standards, professional ethics, and quality assurance requirements.
Essential skills:
- Technical Rehabilitation & Disability Inclusion: Sound knowledge of community-based rehabilitation (CBR), disability inclusion, basic health assessment, assistive devices, referral mechanisms, home modifications, and safeguarding principles to support persons with disabilities and other vulnerable individuals.
- Interpersonal & Relationship-Building Skills: Ability to build trust, actively listen, understand community needs, address concerns, and maintain positive relationships with beneficiaries and stakeholders.
- Planning & Organizational Skills: Ability to organize field activities, coordinate community meetings, manage time effectively, maintain records, and ensure timely completion of assigned tasks. • Cultural Sensitivity & Social Inclusion: Ability to respect local culture, values, and practices while promoting meaningful participation of women, marginalized groups, persons with disabilities, and vulnerable households.
- Behavior Change Communication Skills: Ability to motivate individuals and groups, promote positive practices, and support communities to adopt improved health, nutrition, agriculture, and livelihood behaviors.
- Problem-Solving & Adaptability: Ability to identify field-level challenges, develop practical solutions, adapt approaches based on local contexts, and work effectively in remote and changing environments.
- Documentation & Digital Skills: Ability to collect, verify, and maintain accurate field information, prepare reports, document learning, and use digital data collection tools such as KOBO.
- Driving license: Must have at least one year plus category “A” driving license.
