Posiiton: Social Worker
Job Description
Overall Purpose of the Job
The Social Worker is responsible for providing psychosocial support to patients and their families within the hospital and during field visits. The role focuses on enhancing patient well-being, addressing social and emotional needs, and facilitating access to appropriate care and support services.
Key Task & Responsibilities
- Conduct regular bed-to-bed visits to identify patients in need of psychosocial and emotional support.
- Assist patients in addressing everyday challenges and practical problems related to their health and social circumstances.
- Advocate for patients’ rights and support their overall well-being.
- Work closely with individuals, families, and groups to assess needs and provide appropriate interventions.
- Respond effectively to patients in crisis situations by providing immediate emotional and practical support.
- Provide counseling and emotional preparation for patients undergoing surgical procedures, including pre-operative reassurance.
- Encourage and comfort patients, particularly those undergoing stress or anxiety related to treatment.
- Create awareness among patients and caregivers through educational sessions, including the use of audio-visual materials.
- Conduct and document patient satisfaction surveys, wellbeing assessments, and other required forms.
- Carry out social and financial assessments for patients requiring support from poor patient funds.
- Educate patients and families on health, hygiene, and hospital rules and procedures.
- Counsel patients on the importance of regular follow-up and continuity of care.
- Facilitate spiritual and supportive activities for patients where appropriate.
- Support admitted children (including those with leprosy or other chronic conditions) through basic educational and psychosocial engagement.
- Attend OPD visits and provide support services as required.
- Assist in collecting patient stories and case studies for documentation and reporting purposes.
- Meet patients and families to assess needs and develop individualized support plans.
- Provide ongoing emotional and practical support to patients and caregivers.
- Build trust and rapport with patients to promote confidence and well-being.
- Support patients in developing and maintaining independent living skills.
- Assess patients’ strengths, needs, and available support systems.
- Promote overall physical, emotional, and social well-being of patients.
- Coordinate patient care by liaising with healthcare providers and ensuring continuity of services.
- Support patients in coping with life challenges such as illness, loss, separation, and displacement.
- Conduct regular field visits as required to follow up on patient cases and provide community-based support.
Person Specification
Qualifications and Experience
- Bachelor’s degree in Social work/Psychology or a related field.
- Minimum of 2 years of relevant experience, preferably in a hospital or community health setting.
- Willingness and ability to travel frequently for field visits.
